IMRAN NAZEER, MD, MRCP (UK)
As a Board Certified physician in Internal Medicine and Nephrology, Imran Nazeer, M.D., offers patients the benefits of his extensive experience and specialized training. After receiving undergraduate and graduate degrees from King Edward Medical College in Lahore, Pakistan in the mid-1980’s, he pursued medical studies at the Royal College of Physicians in London, England, and served as a teacher and supervisor at Mayo Hospital in Lahore. Subsequently, Dr. Nazeer worked at Russells Hall Hospital in the West Midlands of the United Kingdom and completed his residency in Internal Medicine at St. Mary’s Hospital in Rochester, New York in 1994. He finished his training in renal medicine at the University of Minnesota, taking his board exams in 1996 and recertified in 2009. The doctor received both clinical and research fellowships from the University of Minnesota in the field of Renal Medicine during his time there. In 1996, he joined the staff of East Texas Nephrology Associates. Also trained as Interventional Nephrologist, he takes care of failing fistulas and graft to prolong the life of a dialysis patient.
Prashant Kumar , MD
Has been with the practice since 2006. As a Board Certified physician in Internal Medicine and Nephrology, he offers patients the benefits of his specialized training. Also trained as Interventional Nephrologist, he takes care of failing dialysis access to prolong the life of a dialysis patient.

Physician with the practice since 2009. As a Board Certified physician in Internal Medicine and Nephrology, he offers patients the benefits of his specialized training in Nacogdoches and San Augustine area.
Della Connor, PhD,FNP-BC,RN
Della Connor was born in South Dakota and raised in Montana. She came to Texas in 1980 to attend college and after that made Texas her home. She earned Bachelor in nursing from Stephen F Austin State University in Nacogdoches, Texas, her Master Degree in Nursing, as a Family Nurse Practitioner at UTMB in Galveston, Texas, and recently earned her PhD in Nursing from the University of Texas at Tyler. She has taught nursing for 11 years at the baccalaureate level. She has been a FNP for the last 12 years practicing in practically every opportunity a FNP has! She has worked part-time since 2007 with our nephrology group, conducting chronic kidney disease educational sessions and rounding on dialysis patients. Her nursing interests include nephrology and dialysis, nutrition and fitness, motivation and coaching for nursing education success. She currently teaches Community Nursing at SFASU.
